Nurse Practitioner

Family Health Teams (FHT) use a patient-centered approach to primary care that brings together doctors, mental health clinicians, nurse practitioners, registered dietitians, registered nurses, registered practical nurses and pharmacists to coordinate the highest possible quality of care for patients- when they need it – as close to home as possible. FHTs, in collaboration with Family Health Organizations, are a key component of Ontario’s healthcare plan to reduce wait times, improve access to primary care and keep Ontarians healthy.

In this position, you will be a member of the Peterborough Family Health Team (PFHT) and work with the Peterborough Community Family Health Organization; a well-established, highly integrated FHO. The Peterborough Community FHO is part of the larger PFHT, encompassing 5 Family Health Organizations in Peterborough County.  This is an exciting opportunity to work within a collaborative environment with 1 family physician as well as mental health clinicians, registered nurses, a registered dietitian and a pharmacist.  This is a TEMPORARY 0.4 FTE position to fill in while the NP is on a temporary leave of absence.

POSITION SUMMARY:

The Nurse Practitioner (NP) will practice within his/her full scope of practice as outlined by the College of Nurses of Ontario.

Major Responsibilities

Including, but not limited to:

  • Complete health assessments including health history and physical examinations
  • Provide wellness care and health screening
  • Diagnose and treat minor illnesses and injuries
  • Screen for and treat chronic illness and monitor stable chronic illnesses
  • Care of episodic illness
  • Determine the need for, and order and interpret reports of X-Rays, ECG’s and diagnostic ultrasound.
  • Monitor the ongoing therapy of clients with chronic stable illnesses by providing pharmacological and counseling interventions.
  • Determine the need for, and the implementation of health promotion and prevention strategies for families and individuals.
  • Use nursing strategies based on evidence based information.
  • Formulate and communicate medical diagnosis or disorder.
  • Consult with a physician in accordance with the standards for physician consultation.
  • Prescribe drugs
  • Gathering and documenting statistical information as required

Nurse Practitioner – 1 YEAR CASUAL, VERY PART TIME CONTRACT for the Trans Care Clinic

FHTs are a key component of Ontario’s health-care plan to reduce wait times, improve access to primary care, and keep Ontarians healthy.

POSITION SUMMARY:

The Nurse Practitioner (NP) will provide transitional care to transgender patients or gender diverse patients as outlined by the College of Nurses of Ontario.  This is a casual part time position, currently we are looking for help every other Tuesday from 4:00 pm to 7:00 pm.  Times and number of shifts may vary slightly depending on need.

Major responsibilities pertaining to the Nurse Practitioner role include, but are not limited to:

  • Complete health assessments including health history and physical examinations
  • Provide wellness care and health screening
  • Determine the need for, and order and interpret reports of X-Rays, ECG’s and diagnostic ultrasound.

Additional responsibilities pertaining to the Trans Gender Care Clinic include:

  • Have an openness to learning about and from patients who identify as Transgender/Gender Non Conforming/Non Binary and their allies and a commitment to providing a welcoming and safe environment for them
  • Achieve familiarity of the Sherbourne Guidelines for Transgender Care
  • Complete a gender history
  • Assess for gender dysmorphia and possible mental health co-morbidities – this task is supported by Mental Health Clinicians on the team
  • Order and interpret baseline and ongoing laboratory tests as per Sherbourne Guidelines
  • Provide education regarding the risks and benefits of hormone and surgical interventions for Transgender patients – obtain informed consent
  • Conduct assessments for surgical referral
  • Navigate the prior approval process with patients to facilitate access to OHIP covered services
  • Provide clinical summaries of care provision to Primary Care Providers in hopes of facilitating their resumption of care for their patients
  • Attend educational opportunities as they arise

Mental Health Clinician – 0.2 FTE, Permanent Part-Time in Apsley

Mental Health Clinician

Bring your advocacy, risk assessment and management skills and knowledge of community resources, psychiatric disorders and human development, family life cycle and systems theory to this 0.2 FTE role based out of Apsley with the Peterborough Family Health Team. Reporting to the Collaborating Physician(s), you will provide psychosocial assessment, short term psychosocial/psychiatric treatment and interventions, psychosocial health education, crisis intervention, and advocacy on behalf of rostered and referred patients as an integral part of a multidisciplinary team. You have a degree in a Social Work preferably at the master’s level, you are registered with the appropriate regulatory body and possess at least 4 years of supervised experience in a counselling environment. Additional training and/or experience in Cognitive Behaviour Therapy, Mindfulness Based Stress Reduction, would be an asset. We are looking for someone with a comprehensive bio-psychosocial approach to mental health assessment, clinical formulation and treatment planning. You must have the flexibility to function within the unique nature of a primary medical care office setting, and be able to participate in, or implement, health promotion, education and self-management strategies.

With more than 115,000 patients in its care, the Peterborough Family Health Team  (FHT) is well positioned to continue to make a significant difference in the health of its community. The organization was a leader in the formation of the family health team model – the answer to a crisis in primary care ten years ago, when one in five residents in the City and County of Peterborough didn’t have access to a family doctor. Since then, the FHT has ensured that an additional 20,000 Peterborough patients now have family doctors and access to team-based primary care. Kinmount – CCM Physician Opportunities

Kinmount is located approx. 2 hours north of Toronto in a modern 7,000sq. ft. Health Centre currently housing 2 doctors, physiotherapy, pharmacy , specimen lab and various allied health services.  This site is eligible for Return of Service and is also wiling to take students for their rural placement.

There are numerous opportunities in Kinmount:

  • Seeking a 3rd physician to join and build a practice from scratch – currently 700-1000 unattached patients in catchment area
  • Seeking a replacement physician for December 2019 for approx. 750 patients
  • Seeking a replacement physician for June 2020 for approx. 1000 patients

All positions have flexible hours but must include one evening per week.  Current physicians are enrolled in a CCM model which also provides a yearly preventative care bonus and capitations payments for the number of patients enrolled. The location qualifies as an under-serviced area and therefore a successful applicant would be eligible for the Northern and Rural Recruitment and Retention grant from the MOH of approx.$83K payable over four years, provided they have not received the grant before, and provided the position is full-time (40 hrs/wk).

The patient population covers all age groups and provides a variety of acute and chronic care experience. The clinic has a full EMR (Accuro) system and there are no on-call requirements.

The general overhead is very competitive at between 18% to 20%

The Foundation provides all medical equipment, fixtures and furnishings and computers.

Physicians need only bring a stethoscope! Four season recreation at the doorstep and much lower living expenses than a large city.

Medical Receptionist-Permanent Part Time (Millbrook)

Medical Receptionist

Position Purpose:

The Medical Receptionist is the first contact in our clinic and is an integral part of a multi-disciplinary team, consisting of two physicians, a nurse practitioner, and other medical staff whose work increases access to, and the quality of, primary health care.

Position Specific Duties:

  • Receive incoming telephone calls
  • Schedule appointments both manually and using PS Suite electronic medical record software
  • Maintain confidential patient health records
  • Receive, sort and send facsimiles
  • Pull patient charts for appointments or follow-up work
  • Perform a variety of confidential duties (e.g., relaying messages, referral workups, processing mail, data entry etc.)
  • Prepare, collect and record payment of third-party bills
  • Assist other team members as needed (e.g., show patients to the examining rooms, tidying rooms, file paperwork/charts)

Hours of Work:  Permanent, Part-Time – 21hrs/week

Monday, Tuesday and Thursday (9-5pm, with a 1-hr unpaid lunch)

There may be an opportunity to increase hours of work through occasional evening (after-hours clinic) work.

Clinic Expectations:

As a member of our health team, it is important that each employee be professional, respectful, and cordial to patients and to each other. As the face of the clinic, it is important that standards including appropriate work attire, punctuality, and telephone availability to patients be upheld.

Qualifications:

  • Demonstrated effective verbal/written communication, interpersonal and public relations and problem-solving skills
  • Proficiency in computer applications: Windows, Microsoft Office programs, etc.
  • Previous medical office and EMR experience preferred
  • Experience in telephone triage and booking appointments
  • Ability to set priorities and organize work accordingly in a busy office setting
  • Post-secondary education in a related field (such as a Medical Secretary course) is considered an asset
