Mental Health Clinician – 1.0 FTE, Permanent

Mental Health Clinician

Bring your advocacy, risk assessment and management skills and knowledge of community resources, psychiatric disorders and human development, family life cycle and systems theory to this 1.0 FTE role based out of Peterborough with the Peterborough Family Health Team. You will provide psychosocial assessment, short term psychosocial/psychiatric treatment and interventions, psychosocial health education, crisis intervention, and advocacy on behalf of rostered and referred patients as an integral part of a multidisciplinary team. You have a degree in a Social Work or Counselling Psychology preferably at the master’s level, you are registered with the appropriate regulatory body and possess at least 4 years of supervised experience in a counselling environment. Additional training and/or experience in Cognitive Behaviour Therapy, Mindfulness Based Stress Reduction, would be an asset. We are looking for someone with a comprehensive bio-psychosocial approach to mental health assessment, clinical formulation and treatment planning. You must have the flexibility to function within the unique nature of a primary medical care office setting, and be able to participate in, or implement, health promotion, education and self-management strategies.

FHTs, in collaboration with Family Health Organizations, are a key component of Ontario’s healthcare plan to reduce wait times, improve access to primary care and keep Ontarians healthy. This is a full time (37.5 hours per week) one year contract position.

POSITION SUMMARY:

The Nurse Practitioner (NP) will practice within his/her full scope of practice as outlined by the College of Nurses of Ontario.

Major Responsibilities

Including, but not limited to:

  • Complete health assessments including health history and physical examinations
  • Provide wellness care and health screening
  • Have an openness to learning about and from patients who identify as Transgender/Gender Non Conforming/Non Binary and their allies and a commitment to providing a welcoming and safe environment for them
  • Diagnose and treat minor illnesses and injuries
  • Screen for and treat chronic illness and monitor stable chronic illnesses
  • Care of episodic illness
  • Determine the need for, and order and interpret reports of X-Rays, ECG’s and diagnostic ultrasound.
  • Monitor the ongoing therapy of clients with chronic stable illnesses by providing pharmacological and counseling interventions.
  • Determine the need for, and the implementation of health promotion and prevention strategies for families and individuals.
  • Use nursing strategies based on evidence based information.
  • Formulate and communicate medical diagnosis or disorder.
  • Consult with a physician in accordance with the standards for physician consultation.
  • Prescribe drugs
  • Gathering and documenting statistical information as required

Qualifications:

Education and Experience:

  • Current RN registration with the College of Nurses of Ontario in the Extended Class
  • Successful completion of the Nurse Practitioner education program
  • Minimum 3 to 5 years in community health nursing in a primary care and or rural setting

Skills and Abilities

    • Proficient in a current word processing program
    • Leadership, critical thinking, organizational and communication skills are required
    • Have expertise in developing and maintaining excellent working relationships with a broad range of individuals and partner organizations

Mental Health Clinician-Group Programming Facilitator, 0.7 FTE Permanent

We are looking for a Mental Health Clinician specifically to deliver Group Programming.  This is a 0.7 FTE permanent position.

This specific clinical role requires group facilitation skills and experience in Mindfulness and Cognitive Behavioural Therapy.   You will provide mental health group programming; you have specific experience in group development, facilitation and curriculum development of groups.  You have experience facilitating mindfulness and CBT orientated groups.   You have a degree in a Social Work or Counselling Psychology preferably at the master’s level, you are registered with the appropriate regulatory body and possess at least 5 years of supervised experience in a counselling environment.    You must have the flexibility to work closely and well with colleagues as you co-facilitate, implement and innovate existing and new mental health programming.  In order to improve service  access to our patients, evening groups are a mandatory part of this position.

Must be able to provide a clear police/vulnerable check.

Quality Improvement Facilitator, part time (15 hours per week), one year contract

We are looking for a: Quality Improvement Facilitator, part time (15 hours per week), one year contract

Reporting to the Medical Director, the Quality Improvement Facilitator will work closely with physicians, team administrators and partners such as Health Quality Ontario to identify and support the implementation of quality improvement initiatives as determined by the strategic and quality improvement plan of the Peterborough Family Health Team (PFHT). This position will also support the evaluation of the effectiveness of health care technologies and services and provide evidence-based recommendations.

This is a part time one year contract position with lots of flexibility.

We are looking for someone who has at least:

  • Undergraduate degree in social or health sciences, policy, health administration, health informatics, or another relevant discipline.
  • A degree resulting in an Ontario college-regulated healthcare professional designation is highly desirable.
  • Experience engaging clinicians, staff, patients and families in design and implementation of delivery of clinical care.
  • Understanding of local and provincial priorities, enablers and barriers to improving quality and delivery of care.
  • Experience as a QI leader, preferred, with experience in, Front line facilitation and coaching of interdisciplinary teams,          Working with provincial health system structures, such as health system funding, integrated care models, etc., Using patient engagement strategies and developing approaches to improve the patient experience, Bibliographic database and Microsoft Office software and applications, including Word, Excel and PowerPoint
  • Additional qualifications, such as: Change Management Certification Knowledge Translation specialist certification, Quality Improvement training (e.g. Lean or Six Sigma), or International Association for Public Participation (IAP2) certification an asset.

An exciting opportunity to join a well established practice and assume care for 500 patients.  This positions would be part of the practice and would receive all compensation for the patients cared for and in return would be responsible for the comparable overhead. This practice is part of a highly integrated Family Health Organization (FHO) and has a flexible start date.  The FHO is part of a larger Family Health Team (FHT), encompassing  5 Family Health Organizations in Peterborough. The practice has a current roster of 1700 patients of all ages and stages.   The practice operates in a collaborative environment with 24 other family physicians who provide extended hours coverage for the entire team.  The practice and entire FHO is fully electronic and has a well established Electronic Medical Record in Practice Solutions (Telus Health).   You will have complete administrative support as well as EMR/IT support.

Kinmount – CCM Physician Opportunities

Kinmount is located approx. 2 hours north of Toronto in a modern 7,000sq. ft. Health Centre currently housing 2 doctors, physiotherapy, pharmacy , specimen lab and various allied health services.  This site is eligible for Return of Service and is also wiling to take students for their rural placement.

There are numerous opportunities in Kinmount:

  • Seeking a 3rd physician to join and build a practice from scratch – currently 700-1000 unattached patients in catchment area
  • Seeking a replacement physician for December 2019 for approx. 750 patients
  • Seeking a replacement physician for June 2020 for approx. 1000 patients

All positions have flexible hours but must include one evening per week.  Current physicians are enrolled in a CCM model which also provides a yearly preventative care bonus and capitations payments for the number of patients enrolled. The location qualifies as an under-serviced area and therefore a successful applicant would be eligible for the Northern and Rural Recruitment and Retention grant from the MOH of approx.$83K payable over four years, provided they have not received the grant before, and provided the position is full-time (40 hrs/wk).

The patient population covers all age groups and provides a variety of acute and chronic care experience. The clinic has a full EMR (Accuro) system and there are no on-call requirements.

The general overhead is very competitive at between 18% to 20%

The Foundation provides all medical equipment, fixtures and furnishings and computers.

Physicians need only bring a stethoscope! Four season recreation at the doorstep and much lower living expenses than a large city.

Build Your Own Practice

Peterborough City and County are on the underserviced list of communities and as such we are currently seeking Primary Care Physicians who are interested in building their own practice as part of one of our five Family Health Organizations (FHO) which are all part of a large community wide Family Health Team (FHT).  We have a variety of business models and locations within our FHO’s which offer everything from turn key practice sites, to completely independent.

The Peterborough Clinic is seeking a highly motivated individual to fill the unique position of medical billing specialist and bookkeeper. Under the direction of the Finance Manager your objective is to review and analyze OHIP billing as well as other accounting duties.

Qualifications:

BILLING

  • 2+ years medical billing experience within a FHO
  • Medical terminology an asset
  • OHIP Reconciling month end reports
  • Follow-up on outstanding claims and resubmissions
  • Strong computer skills/proficiency. Experience with EMR (Accuro) is a strong asset.
  • Ability to work effectively and efficiently without a lot of direction
  • Excellent interpersonal and communication skills (oral and written)

ACCOUNTING

  • The candidate should have college level courses in accounting or business administration, or equivalent business experience, as well as knowledge of bookkeeping and generally accepted accounting principles.
  • Prepare cashbook entries and bank reconciliations
  • Allocate and post financial transactions in accounting software (Sage)
  • Accounts Payable and Accounts Receivable support dealing with vendors or customer invoicing issues as required
  • Prepare checks, payments and bank deposits.
  • Analyze and prepare general ledger entries and conduct monthly account reconciliations

The Peterborough Clinic-Facility Manager

The Manager of Facilities is a key member of The Peterborough Clinic Administration Team reporting to the Chief Administrative Officer (CAO). Directing and motivating the maintenance team, the Manager of Facilities is responsible for overseeing all building maintenance and custodial activities. In addition to regular preventative maintenance and adherence to Health and Safety regulations, the successful candidate will work to fix broken equipment and resolve operational issues as they arise.

The objective of this role is to ensure that our physicians, staff and tenants have the appropriate physical resources available and in good repair to provide outstanding care and customer service to our patients. A solid understanding of plumbing, mechanical and electrical systems as well as carpentry and other trades is required for this position. The ideal candidate will have a natural interest in building systems, a strong work ethic, a sense of urgency and is passionate about safety and customer service.

Responsibilities:

  • Direct team to ensure Clinic building and grounds are clean and safe.
  • Identify areas for improvement and take initiative to resolve issues.
  • Develop maintenance schedules, document maintenance logs and supervise maintenance staff and outside contractors.
  • Respond professionally to calls that could involve equipment malfunction, fire, security or medical emergencies; including after hours when necessary.
  • Perform troubleshooting to solve minor repair issues.
  • Plan and oversee all repair and installation activities.
  • Manage security system.
  • Provide information to the CAO for long term planning.
  • Monitor equipment and supply inventory and place orders when necessary.
  • Monitor expenses and control the budget for maintenance.
  • Manage relationships with contractors and service providers.
  • Ensure health and safety policy compliance.

Skills:

  • Ability to create a maintenance schedule, Ability to troubleshoot
  • Knowledge of maintenance procedures, Knowledge of systems to be maintained (e.g. Electrical, HVAC, roofing, elevator maintenance and repair)
  • Physical agility, manual dexterity and the ability to lift heavy loads
  • Willingness to assist patients when necessary
  • Flexible and adaptable to change; ability to manage in emergency situations.
  • Excellent communications and interpersonal skills
  • Outstanding organizational abilities

Qualifications:

  • Proven experience as a facility manager or similar managerial role.
  • Solid technical understanding of building operation systems, parking systems, and other electrical and mechanical equipment.
  • Working knowledge of medical equipment and supplies is an asset.
  • Diploma or Degree in facility management and/or licensed trade certificate will be an advantage.

The Medical Receptionist provides support with daily activities of a medical office including greeting patients, telephone coverage, appointment scheduling, patient registration, and patient referrals.

Qualifications

  • Must have Medical Receptionist/Ward Secretary diploma or certificate, NO EXCEPTIONS.
