Are you a person with good judgement in the clinical environment, dependable, attentive to detail and the ability to cope effectively with stressful situation?

The Medical Receptionist provides support with daily activities of a medical office including greeting patients, telephone coverage, appointment scheduling, patient registration, scanning medical documents and coordinating patient referrals.

Position required is for a one year contract in Lakefield, ON.

Qualifications
•Must have Medical Receptionist/Ward Secretary diploma or certificate, NO EXCEPTIONS.
•EMR experience preferred
•Well organized, enthusiastic, reliable
•Self-starter with a willingness to learn

Mental Health Clinician – Permanent, 0.9 FTE

Family Health Teams (FHTs) use a patient-centred approach to primary health care that brings together doctors, nurses, nurse practitioners, registered dietitians, pharmacists, mental health clinicians and other health-care professionals and support staff to co-ordinate the highest possible quality of care for patients – when they need it, as close to home as possible. FHTs are a key component of Ontario’s health-care plan to reduce wait times, improve access to primary care, and keep Ontarians healthy.

Mental Health Clinician

Drawing on your 3+ years of clinical experience working with medically ill clients in a health care setting, you will provide bio-psychosocial assessments of patients and their families, psychosocial interventions, health education, crisis intervention, addictions counselling and advocacy on behalf of patients. You hold at least a BSW (MSW preferred) and OCSWSSW registration. Other degrees will be considered, however you must belong to a related professional college. Clinical experience in the community would be an asset, as would additional training and/or experience in any of the following areas: Cognitive Behaviour Therapy, Mindfulness Based Stress Reduction, Addictions, and EMDR. This is a permanent 0.9 FTE or 33.75hours per week role.

NP-Full Time, 1 year contract

Primary Care Nurse Practitioner

The Nurse Practitioner is a Registered Nurse in the Extended Class who uses advanced nursing knowledge and skill to provide optimal care to patients. The Nurse Practitioner promotes judicious use of resources by using good clinical judgment when ordering medications and diagnostic tests, managing chronic and acute conditions and communicating with patients and colleagues.

POSITION SUMMARY:

The Nurse Practitioner (NP) will practice within his/her full scope of practice as outlined by the College of Nurses of Ontario.  This is a full time (37.5 hours per week) one-year contract position.

This position has been mandated by the Ministry and is specifically to address the following category of patients:

  1. Those from the community with no family physician.
  2. Those visiting from outside the community who may or may not have a family physician and require care for acute or episodic events

Major Responsibilities

Including, but not limited to:

  • Practice to the Full Scope of  a Primary Health Care Nurse Practitioner
  • Follows Best Practice Guidelines in order to provide optimal patient care
  • Complete health assessments including health history and physical examinations
  • Provide wellness care and health screening
  • Have an openness to learning about and from patients who identify as Transgender/Gender Non Conforming/Non Binary and their allies and a commitment to providing a welcoming and safe environment for them
  • Diagnose and treat minor illnesses and injuries
  • Screen for and treat chronic illness and monitor stable chronic illnesses
  • Care of episodic illness
  • Determine the need for, and order and interpret reports of Blood work,  X-Rays, ECG’s and diagnostic ultrasound.
  • Monitor the ongoing therapy of clients with chronic illnesses by providing pharmacological and counseling interventions.
  • Determine the need for, and the implementation of health promotion and prevention strategies for families and individuals.
  • Use nursing strategies based on evidence based information.
  • Formulate and communicate medical diagnosis or disorder.
  • Consult with a physician in accordance with the standards for physician consultation.
  • Prescribe drugs as allowed in NP scope of practice
  • Gather and document statistical information as required
  • Manage resources and staff of the NP Clinic  including ordering of supplies and scheduling

Qualifications:

  • Primary Health Care Nurse Practitioner in good standing with the College of Nurses of Ontario
  • Controlled Substances Certification
  • Current CPR course

Education and Experience:

  • Successful completion of a Primary Health Care Nurse Practitioner education program
  • Current registration with the College of Nurses of Ontario in the Extended Class-Primary Health Care
  • Control Substance Certification required
  • Minimum 2 years experience working to full scope as a Primary Health care NP
  • Minimum 5 years experience in an acute/urgent care and/or emergency or outpost practice setting
  • Experience working independently with off-site collaborating physician
  • Management experience an asset
  • Additional training, courses and experience considered an asset ie. IUD insertion; Diabetes certification or training; CBT training; advanced hypertension management

Skills and Abilities

  • Comfortable building a comprehensive EMR database and care plan for a patient and then transitioning that care to a physician when one becomes available
  • Proficient in a current word processing program
  • Leadership, critical thinking, organizational and communication skills are required
  • Have expertise in developing and maintaining excellent working relationships with a broad range of individuals and partner organizations
  • Self-motivated
  • Work in a collaborative practice model
  • Good physical and mental health to meet the demands of the position
  • Must possess a dependable, consistent and reliable work record including attendance at work
  • Availability including some evenings and some weekends

FHO Physician in established FHO/FHT is seeking a partner to assume care for 500 patients

An exciting opportunity to join a well established practice and assume care for 500 patients.  This positions would be part of the practice and would receive all compensation for the patients cared for and in return would be responsible for the comparable overhead. This practice is part of a highly integrated Family Health Organization (FHO) and has a flexible start date.  The FHO is part of a larger Family Health Team (FHT), encompassing  5 Family Health Organizations in Peterborough. The practice has a current roster of 1700 patients of all ages and stages.   The practice operates in a collaborative environment with 24 other family physicians who provide extended hours coverage for the entire team.  The practice and entire FHO is fully electronic and has a well established Electronic Medical Record in Practice Solutions (Telus Health).   You will have complete administrative support as well as EMR/IT support.

About the Peterborough Family Health Team


Kinmount – CCM Physician Opportunities

Kinmount is located approx. 2 hours north of Toronto in a modern 7,000sq. ft. Health Centre currently housing 2 doctors, physiotherapy, pharmacy , specimen lab and various allied health services.  This site is eligible for Return of Service and is also wiling to take students for their rural placement.

There are numerous opportunities in Kinmount:

  • Seeking a 3rd physician to join and build a practice from scratch – currently 700-1000 unattached patients in catchment area
  • Seeking a replacement physician for December 2019 for approx. 750 patients
  • Seeking a replacement physician for June 2020 for approx. 1000 patients

All positions have flexible hours but must include one evening per week.  Current physicians are enrolled in a CCM model which also provides a yearly preventative care bonus and capitations payments for the number of patients enrolled. The location qualifies as an under-serviced area and therefore a successful applicant would be eligible for the Northern and Rural Recruitment and Retention grant from the MOH of approx.$83K payable over four years, provided they have not received the grant before, and provided the position is full-time (40 hrs/wk).

The patient population covers all age groups and provides a variety of acute and chronic care experience. The clinic has a full EMR (Accuro) system and there are no on-call requirements.

The general overhead is very competitive at between 18% to 20%

The Foundation provides all medical equipment, fixtures and furnishings and computers.

Physicians need only bring a stethoscope! Four season recreation at the doorstep and much lower living expenses than a large city.

Peterborough City and County are on the underserviced list of communities and as such we are currently seeking Primary Care Physicians who are interested in building their own practice as part of one of our five Family Health Organizations (FHO) which are all part of a large community wide Family Health Team (FHT).  We have a variety of business models and locations within our FHO’s which offer everything from turn key practice sites, to completely independent.
