Peterborough Community Family Health Team Full Time – December 2011 – March 2012

 

DESCRIPTION OF TEAM:

The Peterborough Networked Family Health Teams’ mission is to improve community access to primary care with an overall mandate to keep our residents healthy, reduce wait times, and provide better access to doctors and nurses.

Primary Health Care Services is the parent organization of the Peterborough Networked Family Health Teams and will be your employer. Our five teams include 83 physicians and we are funded for 21 nurse practitioners, 6.2 dietitians, 13.2 mental health clinicians, 2.7 pharmacists, 24 nurses.

You will be part of the Peterborough Community Family Health Team and will work in collaboration with three female family physicians.

 

POSITION SUMMARY:

This position is 37.5 hours per week and is available from December 8th, 2011 until March 2012. You will work in an all female practice with a high female and paediatric patient population. You will have previous experience or an interest working with these populations. You must be an excellent communicator who is comfortable performing pap smears, discussing birth control as well as participating in chronic disease management programs.

Peterborough Community Family Health Team (Turnbull Medical Building & Times Square locations)
Permanent Full time – 37.5 hours per week
Monday – 8am – 4pm (Times Square)
Tuesday – Friday 9am – 5pm (Turnbull)

Permanent Part Time – Mornings only Monday – Friday

 

DESCRIPTION OF TEAM:

The Peterborough Networked Family Health Teams’ mission is to improve community access to primary care with an overall mandate to keep our residents healthy, reduce wait times, and provide better access to doctors and nurses. Our five teams include 83 physicians and we are funded for 21 nurse practitioners, 6.2 dietitians, 13 mental health clinicians, 2.9 pharmacists, 24 nurses.

This is a unique opportunity for Peterborough, as you would be employed directly by Dr. Jordan Crane who currently has a patient roster of 1600 and is looking to expand his Advance Access practice with the addition of a Nurse Practitioner.

You will be part of the Greater Peterborough Health Team and will work in collaboration with Dr. Crane, registered nurses, mental health clinicians, a registered dietitian, and a pharmacist, to provide comprehensive primary care to the patients of his practice.
