Who’s on a Team?
All Family Health Teams share the same goal: giving you better access to health care and more comprehensive services. In Peterborough we have five family health teams, the Chemong Family Health Team, the Greater Peterborough Family Team, the Medical Centre, the Peterborough Clinic, and the Peterborough Community Family Health Team.
In addition to your family doctor, our teams include the following health professionals:
Nurse Practitioners provide comprehensive assessments; prevent, diagnose and treat illnesses and injuries; and help patients manage their chronic illnesses.

Nurses provide preventive health examinations, as appropriate, and provide health education and monitoring for patients with chronic conditions.

Dietitians provide advice on how nutrition and diet impact your health, as well as providing disease prevention and treatment services for diabetes, heart disease, and other conditions.

Pharmacists provide medication-related education and support to patients with complex illnesses such as heart disease.

Mental Health Clinicians help resolve problems by providing counselling/therapy and by finding required community resources where and when needed.
