Provincial Family Health Teams

A Family Health Team is a concept to primary health care that brings together many different health care providers, to work together and support patients like you.

The “circle of care” idea puts the patient in the centre and surrounds them with professionals such as a family doctor, mental health clinician, nurse practitioner, pharmacist, registered dietitian, and registered nurse. By having access to all these health disciplines, patients receive the very best care, when they need it, and as close to home as possible.
