Who is on a Family Health Organization?

All Family Health Organizations share the same goal: giving you better access to health care and more comprehensive services. The Peterborough Family Health Team is comprised of five Family Health Organizations: the Chemong Family Health Organization, the Greater Peterborough Family Health Organization, the Medical Centre Family Health Organization, the Peterborough Clinic Family Health Organization, and the Peterborough Community Family Health Organization.

In addition to your family doctor, our organizations include the following health professionals:

 

Nurse Practitioners – work independently and in collaboration with your family doctor and other team members to provide appropriate, accessible, quality patient-centered care.

Registered Nurses – can work independently with patients assisting with chronic disease management, well women care, new baby visits and provide preventative care.

Registered Dietitians – translate scientific information about food into useful everyday information for disease management, health and well-being.

Pharmacists – support patients on blood thinner medication through dedicated INR clinics improving outcomes and making it much easier for patients to make the necessary changes to their medication.

Mental Health Clinicians – work along with your family doctor to provide individual and group therahy, in the areas of anxiety, depression, grief, abuse and trauma, relationship and parenting issues, work stress and other life stressors.

Occupational Therapists – work collaboratively with patients to promote health, maintain function and participation in daily occupations, restore independence, prevent disability and allow patients to remain in their homes longer.

Medical Secretaries – work with the team to support them by providing administrative and reception support.
