FHT to QUIT Smoking Cessation Program

The Smoking Cessation program provides a systematic approach to assisting patients when they are ready to quit smoking. Our program is based on best practices adopted from the University of Ottawa Heart Institute (Ottawa Model for Smoking Cessation) and the Centre for Addiction and Mental Health (CAMH), as well as the STOP Program (STOP Study for FHT).

Our specially trained smoking cessation team, including an RN, pharmacist and mental health clinician will:

  • assess your smoking history including nicotine dependency, reasons for smoking and reasons for quitting;
  • provide supportive counselling in one-on-one format;
  • offer free nicotine replacement therapy (NRT) as appropriate;
  • share information on methods for quitting, cessation medications and products;
  • assist you to create a Quit Plan designed to your specific needs; and
  • support you on your journey to be a non-smoker.

If you are a tobacco user and are ready to quit, ask your health care provider how our program might assist you.
